F2T Pre-Approval Form

In order to submit your request to host a F2T 101 Fitness Principles and Practice Course, please ensure that you have all the necessary documents before proceeding. A checklist detailing all required documentation can be found at the link HERE.

All documents must be submitted at the same time and you will be unable to save your progress and resume it at another time.

Please indicate if you have received grant funding to pay for this course.

The information collected in this section will be used for sending notification to your IAFF Local President, and helps to identify and pair your registration with all other documentation being submitted. This information will not be shared outside of the IAFF and Performance Redefined.

Please upload the 360° video of the classroom space. Ensure that desks, tables and chairs are arranged as they will be during the class. Submit the video at the link HERE

Please upload the 360° video of the space to be used for practical sessions. Ensure that the space is empty and free of any obstructing objects. Submit the video at the link HERE

Please indicate your top four preferred course dates. The IAFF makes every effort to accommodate the host's preferred course dates, but cannot offer any guarantee until after the approval process has been reviewed. If you are looking for a date that's not listed (eg. a specific week during the month), fill out all four dates to the best of your knowledge and specify your request in the textbox at the bottom of the page.
